Can a Pseudoaneurysm Cause High Blood Pressure?
While most people associate high blood pressure with lifestyle factors or genetics, certain rare medical conditions can also be to blame—one of them being a pseudoaneurysm.
A pseudoaneurysm, sometimes called a "false aneurysm," occurs when there's a small rupture or leak in an artery wall, leading to blood collecting nearby and forming a bulge. Unlike a true aneurysm, the bulge isn't contained by all layers of the arterial wall, making it more unstable.
One particularly rare but important cause of secondary hypertension is a renal artery pseudoaneurysm. These are uncommon and often go undiagnosed, especially after blunt trauma to the abdomen or lower back. When such an injury damages the renal artery—the main blood vessel supplying the kidney—it can lead to the formation of a pseudoaneurysm.
Why does this matter for blood pressure? The kidneys play a crucial role in regulating blood pressure. A pseudoaneurysm in the renal artery can disrupt blood flow, triggering the renin-angiotensin system, which the body uses to maintain blood pressure. In this case, though, the system overreacts, leading to sustained high blood pressure—sometimes severe and difficult to control with medication alone.
Because renal artery pseudoaneurysms are rare and symptoms can be subtle, they're often under-recognized. Patients with a history of abdominal trauma who develop sudden or worsening hypertension should be evaluated for underlying vascular issues. Imaging studies like CT angiography can help detect these lesions.
When diagnosed early, treatment—such as endovascular repair or stenting—can resolve both the structural problem and, in many cases, the hypertension itself. It’s a reminder that behind some stubborn cases of high blood pressure, there may be an unexpected culprit hiding in plain sight.
