Can a Pseudoaneurysm Rupture? Yes — And It Can Be Dangerous

While pseudoaneurysms are less common than true aneurysms, they’re no less concerning—especially when they form in the visceral arteries, which supply blood to vital abdominal organs like the liver, spleen, and kidneys. These abnormal bulges occur when there’s a tear in the arterial wall, allowing blood to leak into the surrounding tissue and form a contained, pulsating pocket. But unlike a true aneurysm, a pseudoaneurysm isn’t fully enclosed by the arterial wall, making it inherently unstable.

The short answer is yes—a pseudoaneurysm can rupture. When this happens in a visceral artery, the result can be sudden, life-threatening internal bleeding. Although rare, acute free rupture is a medical emergency and carries a high mortality rate if not treated immediately. Patients often present with sudden, severe abdominal pain, hypotension, and signs of hemorrhage, which makes early recognition crucial.

Not all pseudoaneurysms burst, though. Some remain small and stable, particularly if they’ve formed after trauma or invasive procedures like biopsies or pancreatitis. In these cases, doctors may opt for careful monitoring rather than immediate intervention. However, definitive treatment usually involves angiography with arterial embolization—a minimally invasive procedure where a radiologist blocks off the affected vessel using coils or plugs to prevent rupture.

Given the risks, identifying visceral artery pseudoaneurysms early through imaging—such as CT angiography—is key. Once detected, prompt management can mean the difference between a silent resolution and a catastrophic event. While they may be uncommon, the potential for rupture makes these lesions a silent threat worth taking seriously.
