How Big Does an Aneurysm Have to Be to Rupture?

One of the most pressing questions for patients diagnosed with a brain aneurysm is: how big does it have to be before it becomes dangerous? While size is an important factor, it's not the only one—location, shape, growth rate, and individual health all play roles. That said, research suggests that the risk of rupture starts to rise significantly when an aneurysm reaches between 4 and 10 millimeters in diameter.

Studies, including work by Zacks et al., indicate that unruptured aneurysms smaller than 10 mm often carry a relatively low risk of rupture, especially if they’re not growing or causing symptoms. Ultimately, each case is unique. A 7 mm aneurysm in a healthy 30-year-old might be managed entirely differently than a 9 mm one in a 65-year-old smoker with hypertension. The decision to treat—either surgically or with endovascular techniques—depends on a careful balance of risks and benefits.
