How Prostate Removal Affects Men

Undergoing a radical prostatectomy—surgery to remove the prostate due to cancer or other serious conditions—can significantly impact a man’s life, particularly when it comes to sexual function. One of the most common and challenging side effects is erectile dysfunction.

The prostate is surrounded by two delicate bundles of nerves that play a crucial role in achieving an erection. During surgery, even with nerve-sparing techniques, these nerves can be damaged or disrupted. As a result, the vast majority of men experience difficulty getting or maintaining an erection after the procedure. This doesn’t mean intimacy is impossible—many men regain function over time, especially if the nerves were preserved—but recovery can take months or even years.

Age, overall health, and the extent of the cancer all influence the outcome. Younger men with no prior sexual health issues tend to have better recovery odds. Treatments such as oral medications (like sildenafil), vacuum devices, injections, or even penile implants can help manage the condition. Open conversations with a urologist are key to finding the right path forward.

While the physical changes can be hard to accept, it’s important to remember that prostate removal often means beating cancer—and that’s a major victory. Emotional support, patience, and time play just as big a role as medical treatments in regaining confidence and quality of life.
