How Long Does It Take to Lower PSA Levels?
Prostate-specific antigen (PSA) levels are a key marker for monitoring prostate health, especially after treatment for prostate cancer. The time it takes for PSA to drop depends largely on the type of treatment received.
After a prostatectomy, where the entire prostate gland is surgically removed, the primary source of PSA is gone. As a result, levels typically fall rapidly. Most men will see their PSA drop to very low levels—often below 0.1 ng/mL—within just 4 to 6 weeks. In many cases, it becomes undetectable, signaling a successful outcome.
On the other hand, radiation therapy works differently. Instead of removing the prostate, it gradually destroys prostate cells. This means PSA levels decline more slowly. After radiation, it’s common for PSA to take 12 to 18 months—or even longer—to reach its lowest point, known as the nadir. The pace varies from person to person, and occasional small rises (called "PSA flares") can occur during recovery, which don’t necessarily indicate recurrence.
It’s important to remember that every man’s body responds uniquely. Regular follow-up tests are essential to track progress and ensure long-term stability. Patience is key, especially after radiation, where the full effect may not be seen for over a year.
