How to Detect a Pseudoaneurysm: The Role of CT Angiography

When it comes to diagnosing a pseudoaneurysm—also known as a "false aneurysm"—accuracy and speed are crucial. Unlike a true aneurysm, a pseudoaneurysm forms when blood leaks from a damaged artery, creating a contained hematoma connected to the vessel. This condition often arises after trauma, surgery, or catheterization procedures.

One of the most effective and widely used methods for detecting a pseudoaneurysm is computed tomography (CT) angiography. This non-invasive imaging technique provides detailed visualization of blood vessels and surrounding structures. It’s particularly valuable because it combines speed with high diagnostic precision, making it ideal in both emergency and routine clinical settings.

In an unenhanced CT scan—meaning without contrast—pseudoaneurysms may appear as rounded, well-defined collections of blood near the suspected feeding artery. However, the real diagnostic power emerges with contrast-enhanced CT angiography. When contrast dye is injected, the active communication between the artery and the pseudoaneurysm sac becomes clearly visible, often showing a characteristic "to-and-fro" blood flow pattern within the lesion.

CT angiography also helps clinicians assess the size, location, and relationship of the pseudoaneurysm to nearby anatomical structures—key factors in determining whether conservative management, endovascular repair, or surgery is the best course of action.

While ultrasound can be useful in superficial locations (like the femoral artery after catheterization), CT angiography offers a broader, more comprehensive view—especially for deep or internal pseudoaneurysms. It’s this combination of accessibility, sensitivity, and detailed imaging that makes CT angiography a cornerstone in modern vascular diagnosis.
