Is Arterial Plaque a Biofilm?

For years, scientists believed that arterial plaque was primarily the result of cholesterol buildup and inflammation. But recent research suggests there may be more at play—specifically, the presence of bacterial biofilms within the plaque itself.

Studies have found that bacteria in carotid artery plaques often exist not as free-floating microbes, but as structured communities known as biofilms. These microbial colonies coat themselves in a protective matrix, making them harder for the immune system—and antibiotics—to eliminate. Their presence in arterial walls is more than just a curiosity; it could have serious implications for heart health.

When biofilms form within atherosclerotic plaques, they may contribute to instability. The immune system’s response to these persistent bacteria can increase local inflammation, weakening the plaque’s fibrous cap. A thinner cap is more prone to rupture, which can trigger blood clots, leading to strokes or heart attacks.

While not all arterial plaque contains biofilms, their presence—especially in carotid arteries—points to a potentially overlooked factor in cardiovascular disease. This doesn’t mean heart disease is contagious or purely infectious, but it does suggest that chronic, low-grade bacterial presence could be a contributing player in some cases.

Interestingly, the bacteria involved—such as certain oral pathogens—may travel from other parts of the body, like the mouth. This strengthens the well-known link between poor dental hygiene and heart problems. It's another reminder that the body’s systems are deeply interconnected.

Still, much remains to be understood. Are biofilms a cause or a consequence of plaque formation? Either way, their role adds a new layer to how we think about treating and preventing cardiovascular disease—not just with statins and lifestyle changes, but possibly with therapies targeting microbial communities in the future.

For now, the best defense remains a familiar one: a healthy diet, regular exercise, good oral hygiene, and routine medical checkups.
