What Can Be Mistaken for Scoliosis?

Scoliosis, a condition characterized by an abnormal sideways curvature of the spine, is often recognized during adolescence. But it’s not always easy to diagnose correctly the first time. Several conditions can mimic scoliosis, leading to misdiagnosis if a thorough evaluation isn't done.

One common imposter is muscular dystrophy, a group of genetic disorders that cause progressive muscle weakness. As muscles weaken unevenly, posture can become asymmetrical, creating the appearance of spinal curvature. Similarly, cerebral palsy, a neurological disorder affecting muscle control and coordination, can also lead to postural imbalances that resemble scoliosis. In both cases, poor muscle support around the spine may cause a visible lean or uneven shoulders—classic signs parents and doctors often associate with scoliosis.

But the difference is crucial. While scoliosis primarily affects the spine's structure, conditions like muscular dystrophy and cerebral palsy stem from underlying neuromuscular issues. This means treatment approaches vary significantly. X-rays and neurological exams are essential to distinguish between structural spine problems and those caused by muscle or nerve dysfunction.

Other non-spinal factors can also create confusion. Leg length discrepancies, for example, may cause a temporary pelvic tilt, making the spine appear curved. This functional scoliosis often resolves once the root cause—like a shorter leg—is addressed.

Early detection matters, but so does accuracy. If a child shows signs of spinal curvature, a specialist should evaluate not just the spine, but also muscle strength, reflexes, and overall neurological function. Getting the diagnosis right from the start ensures the right treatment path—whether that’s bracing, physical therapy, or addressing a broader neuromuscular condition.
