What Is a Waterfall Breast?

You might have come across the term "waterfall breast" and wondered what it means. It’s not a medical diagnosis, but rather a descriptive phrase—sometimes used informally—referring to a specific change in breast appearance after augmentation. Medically, it’s known as the "waterfall effect," and it describes a condition where natural breast tissue sags or slides over an implant that has remained in place.

This typically happens over time, especially as skin loses elasticity. The implant, often firm and encapsulated due to scar tissue (a condition called capsular contracture), stays fixed in position while the softer, natural breast tissue above it begins to droop. This creates a visual effect where the tissue appears to "cascade" over the upper edge of the implant—hence the name.

It’s more common than many realize, particularly years after breast augmentation. Factors like aging, gravity, weight fluctuations, and implant placement (especially subglandular, above the muscle) can contribute. While it’s not harmful to health, it can affect aesthetic results, sometimes leading to a less-than-ideal contour or an unnatural shelf-like appearance along the upper pole of the breast.

Surgeons today are more aware of this phenomenon and often take preventive steps—such as using anatomical or cohesive gel implants, adjusting placement techniques, or recommending earlier revisions when needed. For patients, staying informed and maintaining regular follow-ups can help manage expectations and outcomes.

The waterfall effect isn’t something every augmented breast will experience, but it’s a reminder that the body changes over time. As with any surgery, long-term results depend on both technique and the natural aging process.
