Understanding End-Stage Scoliosis
End-stage scoliosis refers to a significant progression of spinal curvature that has reached a critical point, often defined by a Cobb Angle exceeding 40°–45° in adolescents and 50°–55° in adults. At this stage, the sideways bend in the spine becomes visibly pronounced, often leading to noticeable postural imbalances—such as uneven shoulders, a prominent rib hump, or a tilted pelvis.
While mild scoliosis may not cause severe issues, end-stage curvature can lead to complications beyond appearance. Some individuals experience breathing difficulties due to reduced chest space, chronic back pain, or fatigue from the extra effort required to maintain posture. In more severe cases, the spine’s rotation can affect the function of internal organs.
For adolescents, progression during growth spurts can accelerate the condition, making early intervention crucial. Surgery—often spinal fusion—is commonly recommended at this stage, not only to halt further curvature but also to improve quality of life and address cosmetic concerns. The decision isn't taken lightly, but when curves exceed 50°, the likelihood of continued progression into adulthood is high, even after growth has stopped.
Adults, on the other hand, may develop or see worsening of scoliosis due to degenerative changes over time. While surgery is considered more cautiously in older patients due to risks, it may still be an option when pain, nerve compression, or loss of function become debilitating.
End-stage scoliosis is more than a spinal deformity—it’s a condition that can impact physical health and emotional well-being. With proper monitoring and timely treatment, however, many people can manage symptoms and maintain an active life.
