The Charlotte Rule: A Smarter Approach to Ruling Out Pulmonary Embolism

When a patient comes to the emergency department with symptoms that could suggest a pulmonary embolism (PE)—like shortness of breath or chest pain—doctors face a critical decision: how to rule it out safely without over-relying on imaging. That’s where clinical decision tools like the Charlotte Rule come into play.

Unlike more complex scoring systems, the Charlotte Rule simplifies the process. It identifies patients for whom PE is considered "unlikely" based on clinical judgment and history. But what makes it stand out is its integration with a sensitive whole blood D-dimer assay. If a patient is deemed unlikely for PE and also has a negative D-dimer result, the rule confidently states that the chance of having a PE is 2% or less.

This combination is powerful. It allows clinicians to avoid unnecessary CT pulmonary angiograms—tests that expose patients to radiation and can lead to false positives. For low-risk patients, this means fewer tests, lower costs, and reduced anxiety, all without compromising safety.

Instead, it emphasizes initial clinical assessment followed by a reliable biomarker test. In real-world settings, this approach streamlines care and reduces over-testing, especially in already busy emergency departments.

Still, like all clinical rules, it’s not for everyone. Patients with a history of PE, those on anticoagulants, or those showing clear signs of deep vein thrombosis should still be evaluated more thoroughly. But for the right patient, the Charlotte Rule offers a clear, evidence-based path to ruling out PE without jumping straight to imaging.
