How Acute Pancreatitis Is Diagnosed

When someone experiences sudden, severe abdominal pain—especially radiating to the back—doctors often consider acute pancreatitis as a possible cause. One of the key ways to confirm this condition is through blood tests that detect specific digestive enzymes released by the pancreas.

During an acute attack, the pancreas becomes inflamed and leaks enzymes into the bloodstream. The two most important enzymes measured are amylase and lipase. Elevated levels of either, but especially lipase, are strong indicators of pancreatitis. Lipase is often preferred because it stays elevated longer than amylase, making it more reliable for diagnosis even if the test isn’t done immediately after symptoms start.

In addition to blood tests, doctors may also use urine tests to detect these enzymes, though blood testing remains the standard. Levels of amylase or lipase that are three times higher than normal are typically considered diagnostic for acute pancreatitis, especially when paired with characteristic symptoms and imaging findings.

It’s not just about the numbers, though. A full diagnosis also involves evaluating the patient’s history, physical exam, and often imaging like a CT scan or ultrasound to look for complications or underlying causes—like gallstones or alcohol use.

Early and accurate diagnosis is crucial. Catching acute pancreatitis quickly helps prevent serious complications such as infection, organ failure, or long-term damage to the pancreas. So while the blood test for amylase and lipase is a cornerstone, it’s most effective when interpreted as part of the bigger clinical picture.
