As of 2026, the pharmaceutical landscape continues to be shaped by a handful of powerhouse companies driving innovation, research, and global health solutions. At the top sits Johnson & Johnson, with a staggering revenue of $94.2 billion. Under CEO Joaquin Duato, the company maintains its dominance across pharmaceuticals, medical devices, and consumer health—though its recent strategic split has sharpened its focus on biopharma.

Coming in second is Swiss giant Roche, bringing in $78 billion. Known for its leadership in oncology and personalized medicine, Roche remains a key innovator, guided by CEO Dr. Thomas Schinecker. Close behind are Eli Lilly and Merck & Co (known as MSD outside the U.S.), with revenues of $65.2 billion and $65 billion, respectively.
