Why Is PDA More Common in Female Infants?
Patent ductus arteriosus (PDA) is a heart condition that affects newborns, particularly premature infants, when a blood vessel called the ductus arteriosus fails to close after birth. While several factors contribute to the development of PDA—such as being born at high altitudes, genetic predisposition, or maternal rubella infection during pregnancy—one consistent observation stands out: PDA occurs about twice as often in female infants as in males.
This 2:1 female-to-male ratio has been well-documented in medical literature, yet the exact reasons remain unclear. Researchers have explored hormonal differences, variations in lung development, and fetal cardiovascular responses, but no definitive biological mechanism has been confirmed. Some theories suggest that female fetuses may have a slower maturation of the ductus arteriosus, making it more likely to remain open after delivery. Others point to differences in prostaglandin metabolism, which plays a key role in keeping the vessel open in utero.
Despite advances in neonatal care, the gender disparity in PDA incidence continues to intrigue scientists. It’s not simply a matter of detection bias—females are genuinely more susceptible, even when accounting for gestational age and birth weight. This pattern underscores the complex interplay between sex and early cardiovascular development.
Understanding why PDA favors one sex over the other could lead to more targeted treatments and better outcomes for affected infants. For now, clinicians remain vigilant, especially in preterm girls, who are at the highest risk. As research progresses, the hope is that these clues will unravel deeper truths about heart development and sex-based differences in neonatal health.
