Can Your Body Reject Sperm? Here’s What You Need to Know
It might sound surprising, but yes—your body can sometimes react to sperm as if it were a foreign invader. This doesn’t mean something is “wrong,” but in certain cases, immune responses can affect fertility. For women, the body may produce antisperm antibodies that attack sperm cells, potentially hindering conception. The same can happen in men, especially after trauma, surgery, or infections that expose the immune system to sperm in ways it normally wouldn’t encounter them.
So how do you know if this is happening? For women, doctors can perform a test called sperm immobilization using a simple blood sample. This checks whether your immune system is producing antibodies that could interfere with sperm movement. Less commonly, a sample of cervical mucus may be analyzed around ovulation to detect immune activity against sperm. These tests aren’t routine—they’re usually considered only after unexplained fertility challenges.
Men can also be tested for antisperm antibodies using the immunobead test, which examines a semen sample. If antibodies are bound to the sperm, especially around the tail, it could impair their ability to swim effectively toward an egg. This test is particularly relevant after procedures like vasectomy reversal, where the immune system might have been exposed to sperm post-surgery.
It’s important to remember that immune-related fertility issues are relatively rare. Most couples trying to conceive don’t need to worry about antibody reactions. But if you’ve been navigating unexplained infertility, talking to your doctor about possible immune factors could offer clarity. While not always treatable, identifying the issue opens the door to options like intrauterine insemination (IUI) or IVF, where sperm bypass natural barriers.
