What Is a Pseudoaneurysm?
Commonly referred to as a "false aneurysm," a pseudoaneurysm is a potentially serious vascular condition that differs from a true aneurysm in both structure and development. While a true aneurysm involves a uniform weakening and ballooning of all three layers of the arterial wall, a pseudoaneurysm forms when there's a rupture or tear in the inner layers of the artery—specifically, the tunica intima and tunica media.
When this breach occurs, blood leaks out but remains contained by the outermost layer, the tunica adventitia, or by surrounding tissues. This trapped blood accumulates, forming a pulsating hematoma that mimics the appearance of a true aneurysm on imaging. Hence, the term “false” aneurysm.
These occurrences are often linked to invasive medical procedures, particularly those involving arterial catheterization—such as cardiac catheterization or angiograms. Trauma, both blunt and penetrating, can also lead to their development. If left untreated, a pseudoaneurysm poses a risk of rupture, thrombosis, or distal embolization.
Diagnosis is typically made using ultrasound Doppler imaging, which can visualize the characteristic “to-and-fro” blood flow pattern within the false lumen. Treatment options vary depending on size and stability, ranging from ultrasound-guided compression and thrombin injection to surgical repair in more complex cases.
Though less common than true aneurysms, recognizing a pseudoaneurysm early is crucial. Its misleading nature means it can go unnoticed until complications arise. Awareness—both in clinical settings and among patients recovering from vascular procedures—can make a significant difference in outcomes.
