What Is a Pseudoaneurysm? Understanding the Difference from Blood Clots

It’s easy to confuse a pseudoaneurysm with a blood clot, but they’re not the same thing. While both involve disruptions in normal blood flow, a pseudoaneurysm—also known as a false aneurysm—is actually a leakage of arterial blood into the surrounding tissue, not a solidified clot.

Unlike a true aneurysm, where the artery wall weakens and bulges, a pseudoaneurysm occurs when there’s a tear or hole in the artery wall. Blood escapes but remains contained by surrounding tissues, forming a cavity that still maintains a direct connection to the artery. This creates a pulsating pocket of blood, which can sometimes be felt during a physical exam.

These often develop after invasive procedures like catheterizations, especially in the femoral artery in the groin, but can also result from trauma or infections. While a blood clot (or thrombus) is a solid mass formed by coagulated blood within a vessel, a pseudoaneurysm is more like a contained rupture—blood surges in and out with each heartbeat, creating a risk of further rupture or compression of nearby structures.

If left untreated, a pseudoaneurysm can grow or even burst, leading to serious internal bleeding. Diagnosis typically involves ultrasound to detect the characteristic "yin-yang" flow pattern in the cavity. Treatment ranges from simple monitoring to ultrasound-guided compression or minimally invasive injections that promote clotting inside the cavity.

So while a pseudoaneurysm isn’t a blood clot, the body often uses clotting as a natural way to seal it off. Understanding this distinction helps clarify how our circulatory system responds to injury—and why timely medical attention matters.
